The College will comply with the provisions of both the Federal Medical Leave Act (FMLA) and Oregon Family Leave Act (OFLA). It is the responsibility of the Office of Human Resources (HR) to establish and communicate a process for employees to follow to be approved for FMLA/OFLA.
It is the responsibility of the manager/administrator to inform the employee of FMLA/OFLA process if they believe the employee may have a qualifying event. It is the responsibility of human resources to process requests and notify employees whether or not the leave qualifies for protective leave under FMLA/OFLA.
It is the responsibility of the employee to notify HR of a potential qualifying event by submitting a Medical Leave Request Form and a Medical Certification of Physician Form as needed for approval by human resources. The FMLA/OFLA request will not be approved if the employee fails to submit the appropriate forms.
