Documentation Criteria
To apply for services through the accessible education services (AES) office, please submit your documentation to our office. You can submit the required documentation by physically dropping it off at the AES office located in the student services office, by faxing it to us at 503-491-7549, or by emailing us at [email protected]. Please make sure to include: your MHCC Student ID Number and a phone number and/or email address where we can reach you with your documentation. Once our office has reviewed and determined eligibility, we will contact you by phone or email to schedule an intake appointment. During this appointment, we will discuss your accommodation request. Documentation Required
If you were on an Individual Education Plan (IEP) or 504 Plan in high school, you may submit a copy of your IEP or 504 Plan as your supporting documentation for our office for review.
Physical Disability
You will need a letter from a medical doctor or nurse practitioner that is signed on letterhead that includes the diagnosis and symptoms, and impact on your learning.
Hearing Impaired or Deaf
You will need to submit your most recent audiogram and report that includes the diagnosis and impact on your learning.
Visually Impaired or Blind
You will need to submit a recent report from a visual exam that includes the diagnosis and impact on your learning.
Psychological Disability or Mental Health Disability
To comply with the guidelines for documentation of a disability, outlined by the Americans with Disabilities Act (ADA), you will need a letter from a psychiatrist, psychologist, therapist, social worker, or medical doctor that is signed on letterhead that includes the diagnosis and symptoms, and impact on your learning.
Learning Disability (LD)
Students should submit their most recent LD Assessment including the following:
Intellectual Assessment Tools
A complete intellectual assessment with all scaled subtest scores and full-scale IQ (FSIQ), Verbal IQ (VIQ), and Performance IQ (PIQ). The preferred instruments are the Wechsler Adult Intelligence Scale-III (WAIS-lll); WAIS-R; and the Woodcock-Johnson Revised Psycho-educational Battery-Revised (WJ-lll). If IQ testing was not completed during your senior year of high school, please submit your most recent IQ testing.
Academic Achievement
A comprehensive academic achievement battery is essential with all standard scores for those subtests reported.
Acceptable instruments include the Woodcock-Johnson Psycho-educational Battery-Revised: Test of Achievement Standard Battery, Wechsler Individual Achievement Test (WIAT); Scholastic Abilities Test for Adults (SATA), and Stanford Test of Academic Skills (STAS).
